We bring together a team of experts, specializing in pulmonology, cardiology and rheumatology, to deliver a full range of services in one convenient location.
Our experts develop a comprehensive treatment plan that is customized to your individual needs. This multidisciplinary approach is ongoing, and progress is regularly reported to your physician so we can ensure that you are receiving the best care possible.
Patients who are not diagnosed with PH will be referred to their primary care physician for follow-up care.

Coordination by Nurse Navigator
At no extra charge, the program’s nurse navigator will coordinate doctor appointments, explain tests and procedures, and offer reassurance when things get complicated. The navigator will help you stay committed to treatment and will keep your primary care physician informed of progress throughout the program to open lines of communication, and more importantly, provide quality outcomes for you.
Symptoms of Pulmonary Hypertension
Often, PH is not diagnosed in a timely manner because its early symptoms can be confused with those of many other conditions. Some symptoms of PH include:
- Breathlessness or shortness of breath, especially with activity
- General fatigue
- Lightheadedness, especially when climbing stairs or when standing up
- Swollen ankles, legs or stomach
- Chest pain, especially during physical activity
Managing Symptoms of Pulmonary Hypertension
Although historically incurable with a poor survival rate, new treatments have significantly improved the prognosis for people with pulmonary hypertension. The symptoms of PH can sometimes be lessened by following these lifestyle changes:
- Don’t smoke
- Rest often
- Stay active
- Avoid high altitudes
- Reduce stress
- Eat a healthy diet
