Following the CDC COVID-19 Booster Shot Guidance patients are invited to schedule their booster below. This page is only for scheduling your first COVID-19 Booster.

Booster Scheduling:

For those ages 18+ AND
Have received their 2nd or 3rd dose primary series at least 5 months ago

For those ages 12 - 17 years old, please schedule your Pfizer booster through MyChart or call 513 86 COVID (513 862 6843).

Please read more about booster vaccines here or contact your primary care physician for guidance. If you have not completed your vaccine series, click here to schedule your first dose. If you need to schedule a second dose, call 513 86 COVID (513 862 6843).
