Continuity of Care
GEROS physicians remain in close contact with residents' primary care physicians, keeping them informed of their patients' progress. In many instances, GEROS Medical Group can become a patient's sole primary care provider.
If hospitalization becomes necessary, our geriatric health professionals ensure continuity of care by communicating closely with the TriHealth hospitalists, who provide care for our patients during their stay at Bethesda North or Good Samaritan hospitals.
The GEROS group can be reached 24 hours a day.
- Calls made between 8:30 a.m. and 4 p.m. weekdays are answered by the office coordinator.
- After-hours and weekend calls are handled through the GEROS answering service.
GEROS is a group of independent, experienced physicians and advanced practitioners who specialize in providing quality primary care to residents of long-term care facilities, assisted living facilities and retirement communities throughout the Greater Cincinnati region. GEROS physicians are not employed by any hospitals, facilities or retirement communities and are solely responsible for all services GEROS provides.
Hospitals and facilities collaborate with GEROS to enhance the continuity of care for long-term care patients during inpatient stays. If hospitalization becomes necessary, GEROS physicians and advanced practitioners work closing with hospitalists to provide care for patients during their stay.
