Medical Student Rotation Application

Application and Contact Information

Medical students must currently be enrolled in an accredited medical college. In order to apply for our program, please submit the following:

• Transcripts
• Immunization records
• USMLE Scores (Step I and II)
• Proof of Liability Insurance
• Letter from the dean of your school regarding academic standing
• CV or resume
• Completed (medical student) application form
