TriHealth Named American Medical Group's Acclaim Award Honoree

The American Medical Group Association (AMGA) has named TriHealth one of three honorees of the 2012 AMGA Acclaim Award, given annually to medical groups for quality improvements in patient care.

TriHealth was recognized by AMGA for submitting a presentation entitled: “Patient Centered Medical Home & Clinical Data Warehouse: Shifting the Paradigm toward Population Health.”The award will be presented to TriHealth at AMGA’s Institute for Quality Leadership Annual Conference, being held October 3-5, 2012 in Baltimore, Maryland.

“TriHealth practices have developed a national reputation for innovation in how we deliver patient-centered primary care,” said Will Groneman, Executive Vice President, System Development for TriHealth. “Our doctors have redesigned their practices to be more responsive to their patient needs. While our physicians continue to provide traditional episodic care for their patients, they now also focus on what it will take to keep their patients healthy. To maximize these efforts, TriHealth developed the TriHealth Clinical Data Warehouse which pulls information from the patient’s electronic medical records and presents to the doctors and their staff so they can proactively work with their patients on improving their health. This is groundbreaking work. We are thrilled the AMGA has recognized our achievements.”

Led by over 30 primary care practices, TriHealth’s clinical data warehouse has shifted the paradigm toward population health by arming physicians and staff with comprehensive, reliable data, empowering them to partner with patients to achieve optimum outcomes.

The Acclaim Awards are given to groups that demonstrate progress in achieving the six aims of safe, effective, patient-centered, timely, efficient and equitable care, as outlined by the Institute of Medicine. Applications are evaluated in a blinded review process conducted by nationally recognized leaders in healthcare quality.

“These organizations demonstrated a monumental leap forward in providing patients the health care they need, want, and deserve,” said Donald W. Fisher, Ph.D., AMGA’s president and chief executive officer and AMGF secretary/treasurer.  “We salute the dedication of the organizations’ leadership and healthcare providers in improving patient outcomes, efficiency, coordination, and organizational and clinical integration. Their ongoing efforts will ensure that their patients will receive quality, value-based care throughout their lives.”

Practices sharing in the award include: Group Health, Queen City Physicians, Health First, and other TriHealth Physician Partners.

Thank you to a few of the participating physicians and practices who have contributed to the clinical data warehouse and patient-centered medical home efforts over the last few years and made this award possible.
