Apply for Financial Assistance

At TriHealth, we understand that unexpected medical expenses can be overwhelming. If you're having trouble paying all or part of your hospital bill, we’re here to help. You may qualify for financial assistance through federal, state, or hospital programs—and we can guide you to additional resources that may ease the burden.

TriHealth Financial Assistance is based on income and household size. Details can be found in the Plain Language Summary. To find out if you’re eligible, simply complete our financial assistance application.

You can apply for or obtain an application in several convenient ways:

• Through MyChart
  • Select “Billing”
  • Select “Financial Assistance”

• At any Admission office, Registration desk, Financial Counseling office, or TPP/TriHealth Physician Partners front office.
• By downloading the financial assistance application.

To determine your eligibility for Financial Assistance, you must submit:

• A completed Financial Assistance Application
• Proof of income for the three (3) months prior to the date of service for which you are requesting assistance

Applying through MyChart is a convenient way to complete and submit your Financial Assistance Application. Through MyChart, you can:

• Complete and submit your application online
• Upload required proof of income documentation
• Track the status of your application after all required documentation has been submitted

Scan the QR code to sign in to MyChart or create a new MyChart account.

If you do not have an active MyChart account, you can create one using the QR code above or visit any Registration Desk to request an activation code.

If you choose to complete a paper application, your completed application and supporting documentation may be mailed, emailed, faxed, or delivered to one of our Financial Counseling offices.
