This Privacy Statement describes how TriHealth, Inc., and its subsidiaries and affiliates (collectively, “TriHealth”, “we”, “us” or “our”) will collect and use information that you provide, and information collected from our website, mobile applications and data transmitted to TriHealth (the “Services”). It also describes the choices you have regarding the use of your information and how to update your choices.
What is Covered by the Privacy Statement?
This Privacy Statement applies to the Services linked to this Privacy Statement. It applies to information TriHealth collects, uses and shares through any of the Services. This Privacy Statement also applies to information shared with us by our approved partners and other third-party sources. This Privacy Statement does not apply to information collected from you offline or through websites and other online services that do not link to this Privacy Statement, including third-party websites to which our online services may link, or to Protected Health Information (PHI) as defined by the Health Insurance Portability and Accountability Act (HIPAA). TriHealth’s use and disclosure of PHI is fully described in our Notice of Privacy Practices.
How We Collect and Use Personal Information
When you view or interact with our Services, we may collect information about you, how you engage with our Services, and the devices you use to view or interact with our Services for the purposes we describe below. This includes information you provide to us in connection with your use of the Services and information collected automatically when you view and/or interact with our Services.
Some of the information we collect may be personally identifiable information, which includes, but is not limited to, your name, email address, mailing address, or mobile phone number. We do not collect information that would personally identify you unless you choose to provide it or as otherwise described in this Privacy Statement. You can access and browse our Services at any time without providing any personally identifying information.
TriHealth also aggregates non-personally identifiable information to understand how the TriHealth.com site and its advertisements are being used and can better serve its users. We collect Internet Protocol (IP) addresses of devices that access the site. IP addresses do not personally identify an individual since multiple individuals can access the same computer. The information collected on the site, including general geographic location, the site or advertisement you came from, your device type, and pages you visited, is used to improve site experiences. Aggregate data will only be shared with TriHealth’s permission.
We may combine this information with other information that we already have about you.
Information You Provide to Us
We collect information you provide directly to us, such as when you create or modify your account or user preferences, sign up for a digital newsletter, contact us, respond to a survey, or otherwise communicate with us. This information may include your name, email address, mobile phone number, mailing address, insurance information, payment information, personal health information (which is not protected by HIPAA), survey responses, information entered into the forms found on our Services, and other information you choose to provide through use of our Services.
The information you provide to us may be used to provide and improve our Services; contact you; provide you with products, services or information that you request from TriHealth; and for any other purpose for which you provided it. We may further use your information to prevent potentially prohibited or illegal activities and in accordance with our Terms and Conditions; and for any other purposes disclosed to you at the time we collected your information or obtained it with your consent.

Information Collected Automatically
We may automatically collect additional information about when you use our services, including pages you visit and view within the Services, your geographic location based on your Internet Protocol (IP) address, promotions and services you viewed and other actions. This information is collected for the purpose of improving your visit to our Services by providing interest-based content. For example, we may collect information about your geographic location and a specific interest, based on topics you have reviewed on our Services, so that we can highlight a program or resource close to your location. The data may also be used to provide interest-based content for communications for which you have opted in. We may collect technical data to address technical problems and improve our Services.

Information Collected by Third-Party Tools
TriHealth also uses technologies such as cookies and pixel tags to analyze site traffic, personalize content, and create more relevant advertisements. This information is also shared with trusted advertising and analytic partners.
Your Control Over Information
There is a variety of tools available to you to manage cookies and similar technologies that collect information related to your use of the Services, including:
Web browser controls to block and delete cookies, or to adjust your tracking preferences; and By visiting the Digital Advertising Alliance or Network Advertising Initiative respective websites to opt-out of receiving third-party participant companies advertising.
For cookies and similar technologies used on our mobile applications for applicable targeted advertising activities, use the settings in your mobile device(s) to opt out of having your information collected and used by us and our digital marketing partners.
You may opt-out of Facebook Pixel, Flashtalking and Google Analytics by visiting Facebook, Flashtalking, and Google Analytics Opt-Out Browser Add-On.

Information We Share
We may share the information described in this Privacy Statement or as described at the time the information is collected.
We may share information with a subsidiary, or affiliate entity within TriHealth. Any organization to which we provide this information is required to keep it confidential and comply with this Privacy Statement.
We may share information with service providers and business partners who perform a service for us. For example, we may provide information as necessary to companies that host our Services or send communications on our behalf. These service providers may have access to information about you if it is needed to perform the services, but they are not authorized by us to use or disclose the information except to perform services or as required by law and they are required to maintain the confidentiality of the information.
We will not sell or exchange this information, nor will we authorize any of our service providers or business partners to use this information for their own commercial purposes without your consent.
Required by Law Disclosure
We may disclose information if required to do so to comply with a law or process that requires the disclosure. We may also disclose information as part of an investigation or enforcement action relating to improper or illegal conduct in connection with our Services.
TriHealth may share your information with our business partners, such as local advertisers who pay for banner ads on our Services. These business partners may use the information received by them to contact you to offer you certain opportunities, products, services, or promotions. Once your information has been shared with a business partner, it is no longer subject to the control of TriHealth or subject to this privacy statement, however TriHealth requires its business partners to maintain appropriate data security.
We have implemented technical safeguards to protect the information we collect and use from theft, loss and misuse. However, no data transmitted over the internet is guaranteed to be 100 percent secure.
The Services offer opportunities to communicate through email. Because regular email is not encrypted, the possibility exists that unauthorized individuals may intercept email messages. TriHealth is not responsible for the privacy and security of email messages except those stored in our system.
Our website may contain links to external third-party websites. TriHealth is not responsible for the content or privacy policies of the third-party websites.
California Privacy Rights
California residents have the legal right to request in writing from businesses with whom they have an established business relationship: (a) a list of the categories of personally identifiable information such as name, email, and mailing address, and the type of services provided to the customer that a business has disclosed to third parties (including affiliates that are separate legal entities) during the immediately preceding calendar year for the third parties’ direct marketing purposes; and (b) the name and address of all such third parties. Visiting Our Services from Outside The United States
The Services are located in and directed only at users in the United States.
