Financial Arrangements

In an effort to keep healthcare costs down, payments for copays are expected at the time services are rendered. If no insurance is available, you will be required to pay at the time services are rendered. For your convenience, we accept cash, check, and all major credit cards.Need help? Fees

Fees for obstetrical care are billed globally and include routine prenatal care (normally 13 to 15 visits), vaginal or caesarean delivery and routine postpartum care following delivery (normally includes 1 to 2 visits depending on type of delivery). Additional fees may be assessed for any other test or procedure rendered during your obstetrical care. We offer various payment plans for these services.

Surgical fees may vary depending on the procedure performed. Any required pre-operative testing or facility fees are not included in our fee and will be billed by other service providers. You may be required to pay copays and deductibles before services are rendered. If you are unable to pay any remaining balances after surgery, we offer various plans to assist you with payment.

Forms

As a service to you, we will complete forms for a nominal fee. The charge for form completion is $25.00 for the first form, $20.00 for the second form, and $15.00 for each additional form. Payment is required at the time the forms are submitted. Payment can be made in check, cash or any major credit card.

Insurance Claims

As a courtesy, we will initiate a claim to your insurance company on your behalf. We accept most insurance plans. We do our very best to stay current on our accepted plans, but due to constant changes beyond our control, we recommend that you contact your insurance to verify your coverage for services. As a patient, it is ultimately your responsibility to make sure you are receiving services from a provider in your plan, and not the responsibility of our office. Please keep in mind, your medical insurance may not pay in full for all services rendered; therefore, you are responsible for any remaining balances after insurance processes your claim. As the patient, it is your responsibility to provide updated insurance information to our office for proper claim submission. If you have questions regarding the processing of your claim, please contact your insurance company. If you have questions regarding a bill, please contact one of our billing specialists at (513) 569-6117 and they will be happy to assist you.
