Heart Failure

Heart failure is a condition in which the heart is not able to pump blood effectively to support the needs of the body. In advanced heart failure, the late stage of heart failure, standard therapies are no longer effective.

The variety of treatments utilized by our medical professionals to help patients as they reach the advanced heart failure stage include medications, implanted devices, as well as surgical options.

  • Protocolized rapid multidisciplinary GDMT titration with APPs, pharmacists, and HF MDs
  • Remote heart failure monitoring: LifeVest®, PA hemodynamic monitoring; CardioMEMS, HFMS, Beacon/HeartLogic ICD/CRT monitoring
    • CardioMEMS; PA hemodynamic monitoring.
